Job Description
Make a meaningful impact in patient health by performing in-home risk assessments for Medicare Advantage members throughout Hutchinson County, TX. As a Nurse Practitioner in this per diem position, you will conduct comprehensive evaluations for individuals in their own homes, supporting care continuity and promoting improved patient outcomes.

Key Responsibilities:
- Conduct annual health risk assessments for Medicare Advantage plan members in local communities such as Stinnett, Borger, and Fritch
- Review and evaluate patients' medical history, current conditions, and medication use
- Educate members on chronic diseases, medication adherence, and wellness strategies
- Assess for potential healthcare gaps and offer actionable recommendations to help address these needs
- Focus solely on assessmentno treatment plan creation or medication prescribing required
Qualifications:
- Current and unencumbered Nurse Practitioner license in Texas
- Active BLS/CPR certification
- Board Certification through AANP or ANCC
- MSN degree from an accredited nursing program
- At least 6 months of recent experience as a Nurse Practitioner is preferred
