Job Description

Take on an impactful role delivering in-home risk assessments for Medicare Advantage plan members across Dyer County, TN. As a Nurse Practitioner specializing in home health, your responsibilities will include traveling to patient residences in areas such as Dyersburg, Newbern, Trimble, and nearby communities to conduct comprehensive health assessments. Each visit lasts about 45 minutes and may range from annual assessments to post-discharge evaluations.

Schedule:

  • Flexible shifts available between Monday and Sunday
  • Design your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Complete annual home health risk assessments for Medicare Advantage members
  • Review and evaluate patients prior and current medical histories, conditions, and medications
  • Deliver education to patients about their health conditions and medication management
  • Identify care gaps and offer recommendations to address them
  • No requirement to create treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner License in Tennessee (TN)
  • Current CPR certification (BLS)
  • Board certification through AANP or ANCC
  • Minimum of 6 months current NP experience preferred
  • MSN degree from an accredited nursing program

Youll be compensated between $80-$90 per evaluation, with an additional mileage stipend and payment for administrative time.
