Job Description

Advance your career as a Nurse Practitioner specializing in home health by conducting risk assessments for Medicare Advantage plan members throughout Allegany County, NY. This per diem opportunity allows for a highly flexible schedule, ideal for professionals seeking autonomy and control over their workdays. Youll conduct in-home visits across communities such as Wellsville, Alfred, Cuba, Caneadea, and other nearby areas.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Design your own schedule based on your availability
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Complete comprehensive annual home health risk assessments for plan members
  • Review and evaluate patients prior and current medical histories, conditions, and medication regimens
  • Provide clear education to patients on their conditions and medications
  • Identify any care gaps and offer recommendations to help close those gaps
  • No responsibility for creating treatment plans or prescribing medications

Who Were Looking For:

  • Active Nurse Practitioner license in NY
  • Valid CPR certification (BLS)
  • Board Certification via AANP or ANCC
  • Preferred: At least 6 months of current NP experience
  • Master of Science in Nursing (MSN) from an accredited institution
