Job Description

This per diem opportunity offers flexibility, autonomy, and the chance to work with diverse Medicare Advantage plan members throughout Allegany County, NYincluding locations such as Wellsville, Alfred, Cuba, and Caneadea.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule to fit your lifestyle
  • Minimum weekly or bi-weekly hourly commitment

Responsibilities:

  • Conduct comprehensive annual or post-discharge home health risk assessments in patients homes
  • Evaluate prior and current medical history, medical conditions, and medication use
  • Provide clear patient education regarding medical conditions and prescribed medications
  • Identify gaps in care and recommend appropriate resolutions to improve health outcomes
  • No requirement for treatment plan creation or prescription writing

Qualifications:

  • Active Nurse Practitioner license in New York
  • Current CPR (BLS) certification
  • Board Certification through AANP or ANCC
  • MSN degree from an accredited nursing program
  • At least 6 months of recent Nurse Practitioner experience preferred

This role offers a competitive rate per completed evaluation ($80-$90), including mileage stipend and compensation for administrative time.
