Job Description

A dynamic per diem opportunity awaits experienced Nurse Practitioners skilled in home health assessments. This role focuses on conducting comprehensive home health risk assessments for Medicare Advantage plan members in Allegany County, NY, including towns such as Wellsville, Alfred, Cuba, Caneadea, and surrounding areas. As a valued healthcare provider, you will make a critical impact by ensuring patients receive essential preventative health evaluations from the comfort of their homes.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Build your own schedule to match your availability
  • Minimum hourly commitments may be required on a weekly or bi-weekly basis

Key Responsibilities:

  • Complete annual home health risk assessments during home visits, typically lasting about 45 minutes per assessment
  • Evaluate patients current and prior medical history, document ongoing conditions, and review medication regimens
  • Educate patients regarding their medical conditions and medication usage
  • Identify gaps in care, and offer recommendations to improve patient outcomes
  • No treatment planning or medication prescribing required focus is on assessment and education

Qualifications:

  • Active Nurse Practitioner License in NY
  • Current CPR (BLS) certification
  • Board Certification through AANP or ANCC
  • Master of Science in Nursing (MSN) from an accredited educational institution
  • At least 6 months of current NP experience preferred
