Step into a pivotal role as a Nurse Practitioner conducting home health risk assessments within Chautauqua County. This per diem opportunity is ideal for compassionate practitioners seeking flexibility and meaningful patient engagement in varied locations, including Jamestown, Dunkirk, Chautauqua, and Mayville.
Schedule:
- Flexible shifts available any day, Monday through Sunday
- Build your own schedule based on personal and professional commitments
- Choose your minimum weekly or bi-weekly hours for optimal work-life balance
Key Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage health plan members in their residences
- Review and evaluate patients prior and current medical history, existing conditions, and medication usage
- Provide patient education regarding medical concerns and prescribed medications
- Identify care gaps and offer actionable recommendations for improved health outcomes
- Note: This position does not require developing treatment plans or prescribing medications
Desired Qualifications:
- Valid Nurse Practitioner License in New York State
- Current CPR certification (BLS)
- Active Board Certification through AANP or ANCC
- Minimum of 6 months experience as a Nurse Practitioner is preferred
- MSN degree from an accredited nursing school
As part of this role, you will be compensated at $80$90 per evaluation, with additional mileage reimbursement and compensation for administrative duties.
