Job Description

Provide compassionate, comprehensive care as a Nurse Practitioner conducting home health risk assessments for Medicare Advantage plan members across Chautauqua County. In this per diem role, you'll make a meaningful impact by evaluating patients in their own homes, with assignments that may include annual health assessments or post-discharge visits. Each evaluation typically lasts about 45 minutes and is designed to ensure members receive the highest standard of personalized health attention.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Design your own work schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Perform thorough annual home health risk assessments for Medicare Advantage plan members
  • Review and evaluate medical history, current conditions, and medication regimens
  • Provide clear patient education regarding medical conditions and medications
  • Identify gaps in care and communicate recommendations for follow-up or resolution
  • Travel throughout Chautauqua County for in-home evaluations, including areas such as Jamestown, Dunkirk, Chautauqua, and Mayville
  • No requirement to develop treatment plans or prescribe medications

Required Qualifications:

  • Active Nurse Practitioner license in New York
  • Board Certification through AANP or ANCC
  • Current CPR (BLS) certification
  • Master of Science in Nursing (MSN) from an accredited institution
  • At least 6 months of recent Nurse Practitioner experience is preferred

In this rewarding per diem position, you will receive compensation of $80$90 per evaluation, in addition to a mileage stipend and administrative time.
