Job Description

Work directly with Medicare Advantage plan members, providing high-quality home health risk assessments throughout Stephenson County, Illinoisincluding Freeport, Lena, and Orangeville. Enjoy autonomy over your schedule and the opportunity to positively impact patient outcomes.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule
  • Choose a minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Perform annual and post-discharge home health risk assessments in patient homes
  • Evaluate both current and prior medical history, and assess medication management
  • Provide tailored education regarding medical conditions and medications
  • Identify gaps in patient care and recommend next steps for resolution
  • Visits are focused on assessment; no medication prescribing or treatment planning required

Qualifications:

  • Active Nurse Practitioner license in Illinois
  • Current CPR certification (BLS)
  • Board Certification through AANP or ANCC
  • Minimum six months of NP clinical experience preferred
  • MSN degree from an accredited nursing school

Compensation and Perks: For each evaluation, youll receive compensation ranging from $80 to $90, along with a mileage stipend and payment for administrative time.
