Take on a vital role delivering high-quality home health care as a Nurse Practitioner providing risk assessments throughout Ocean County, NJ. Your assessments may occur in Lakewood, Toms River, Brick, or the surrounding areaseach shift assigned according to your selected availability.

Schedule:

• Flexible shifts available seven days a week
• Design your own work schedule
• Commit to hourly minimums on a weekly or bi-weekly basis

Key Responsibilities:

• Conduct thorough annual home health risk assessments for Medicare Advantage members
• Evaluate patients medical histories, current conditions, and medication regimens
• Educate individuals on their health conditions and proper medication use
• Identify care gaps and recommend solutions, supporting preventive health
• No requirement to create treatment plans or prescribe medications, allowing focus on thorough assessment and education

Qualifications:

• Active Nurse Practitioner license in New Jersey
• BLS (CPR) certification up to date
• MSN earned from an accredited school of nursing
• Board certified via AANP or ANCC
• Minimum of six months recent NP experience preferred

For your expertise, you will receive compensation of $80$90 per evaluation, supplemental mileage stipend, and additional administrative time.
