Step into a per diem opportunity that prioritizes flexibility while making a meaningful impact in home health care. As a Nurse Practitioner performing home health risk assessments, you will work with Medicare Advantage plan members throughout locations in Ocean County, NJincluding Lakewood, Toms River, and Brickdelivering crucial assessments and supporting members overall well-being.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own workdays and hours
- Minimum weekly or bi-weekly hourly commitment required
Responsibilities include:
- Conducting home health risk assessments, typically 45 minutes per visit
- Reviewing and documenting prior and current medical histories and medication use
- Educating patients on their chronic conditions and prescribed medications
- Identifying gaps in care and recommending necessary actions for care coordination
- Performing a variety of visit types, including annual health assessments and post-discharge evaluations
- No development of treatment plans or prescription of medications
Qualifications:
- Active Nurse Practitioner license in New Jersey
- Current CPR certification (BLS required)
- Board Certification through AANP or ANCC
- MSN degree from an accredited nursing program
- At least six months of recent NP experience preferred; motivated new graduates may also be considered
Compensation for this position ranges from $80 to $90 per evaluation, with an additional stipend for mileage and compensated administrative time.
