Experienced Nurse Practitioners are needed for per diem home health risk assessments throughout Monmouth County, NJ. If you thrive in autonomous environments and enjoy meaningful patient interactions, this opportunity offers a rewarding blend of flexibility and impactful care.
Schedule:
- Flexible shifts available Monday through Sunday

Key Responsibilities:
- Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members
- Perform thorough evaluations of prior and current medical history, chronic conditions, and medication regimens
- Provide critical education to patients regarding medical conditions and proper medication usage
- Identify care gaps and offer evidence-based recommendations for further care
- Visits may include annual health evaluations or post-discharge assessments; each assessment is typically 45 minutes
- No treatment plan creation or medication prescribing is required
Qualifications:
- Active Nurse Practitioner license in New Jersey
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- Preferred: At least 6 months experience as a Nurse Practitioner
- Master of Science in Nursing (MSN) from an accredited institution
This per diem, field-based position offers the ability to provide critical preventive care and support the health of Medicare Advantage members in residential settings. Travel within Monmouth County is required, including areas such as Middletown, Howell, and Marlboro.
