Advance your career in an impactful per diem Nurse Practitioner opportunity focused on delivering quality care in a home health setting. This role is dedicated to performing home health risk assessments for Medicare Advantage plan members in Spotsylvania County, VAincluding areas such as Spotsylvania Courthouse and Lake Wilderness. Each visit averages approximately 45 minutes, encompassing services such as annual health assessments and post-discharge follow-ups.

Schedule:

• Flexible shifts available Monday through Sunday
• Self-schedulingset your own hours
• Choose a minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct comprehensive annual risk assessments in patient homes
• Evaluate prior/current medical history, conditions, and medication usage
• Deliver education regarding health conditions and prescribed medications
• Identify care gaps and recommend appropriate resources
• No requirement to develop treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in Virginia
• Current CPR (BLS) certification
• Board Certification through AANP or ANCC
• MSN degree from an accredited nursing program
• At least 6 months of recent NP experience preferred

Compensation for this per diem role is provided per completed evaluation, with $80-$90 per visit in addition to a mileage stipend and administrative time reimbursement.
