Advance your Nurse Practitioner career with a highly flexible per diem opportunity focused on home health risk assessments for Medicare Advantage plan members. In this role, you will visit patients in their homes within Vance County, providing vital annual health assessments or post-discharge evaluations. Each home visit lasts approximately 45 minutes and takes place in locations such as Henderson, Kittrell, Middleburg, and surrounding areas. Schedule:

• Flexible shifts available Monday through Sunday
• Create your own schedule to fit personal and professional commitments
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Perform comprehensive home health risk assessments for Medicare Advantage members
• Review and document medical history, existing health conditions, and medication usage
• Deliver clear education to patients on their conditions and prescribed medications
• Identify and report any care gaps with appropriate recommendations to care teams
• No requirement to create treatment plans or prescribe medications

Qualifications:

• Current Nurse Practitioner license in North Carolina
• MSN from an accredited nursing institution
• Active certification from AANP or ANCC
• Current CPR certification (BLS)
• At least 6 months of current NP experience preferred

This position offers compensation of $80$90 per completed evaluation, with a mileage stipend and additional pay for administrative time, ensuring your effort and expertise are recognized and valued.
