This per diem opportunity focuses on supporting Medicare Advantage plan members throughout Robertson County. As a Nurse Practitioner in this role, you will work independently and enjoy the flexibility of self-scheduling.
Flexible Schedule:
- Shifts available any day of the week, Monday through Sunday

- Choose a weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct thorough annual and post-discharge home health risk assessments at patients residences
- Review and evaluate medical history, chronic conditions, and current medication regimens
- Educate members about their diagnoses and prescribed therapies during each visit
- Identify and document any potential gaps in care, offering actionable recommendations
- No requirement to create individualized treatment plans or prescribe medication
Qualifications:
- Valid Nurse Practitioner license in Texas
- Current CPR (BLS) certification
- Board certification from AANP or ANCC
- Minimum of six months recent experience as a Nurse Practitioner preferred
- Master of Science in Nursing (MSN) from an accredited institution
For each completed evaluation, compensation ranges from $80 to $90, with an additional mileage stipend and compensation for administrative activities.
