Step into a rewarding, flexible role performing home health risk assessments as a Nurse Practitioner for Medicare Advantage plan members throughout Williamson County, TN. Use your advanced clinical skills in a per diem position designed for autonomy and positive patient impact, with the majority of visits taking place in residences across Franklin, Brentwood, and Spring Hill.

Schedule:

• Flexible shifts available between Monday and Sunday
• Design your own schedule with a minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Complete comprehensive annual health risk assessments in the home setting
• Review and document prior and current medical histories, existing conditions, and medication usage
• Educate members on their medical conditions and medications, encouraging informed self-care
• Identify and discuss gaps in care, providing appropriate recommendations
• Focus strictly on assessmentsthere are no duties involving development of treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner license in Tennessee
• Current CPR certification (BLS required)
• Board Certification via AANP or ANCC
• Minimum of 6 months nurse practitioner experience is preferred
• MSN degree from an accredited nursing school
