Step into a rewarding, flexible role performing home health risk assessments as a Nurse Practitioner for Medicare Advantage plan members throughout Williamson County, TN. Use your advanced clinical skills in a per diem position designed for autonomy and positive patient impact, with the majority of visits taking place in residences across Franklin, Brentwood, and Spring Hill.
Schedule:
- Flexible shifts available between Monday and Sunday
- Design your own schedule with a minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete comprehensive annual health risk assessments in the home setting
- Review and document prior and current medical histories, existing conditions, and medication usage
- Educate members on their medical conditions and medications, encouraging informed self-care
- Identify and discuss gaps in care, providing appropriate recommendations
- Focus strictly on assessmentsthere are no duties involving development of treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in Tennessee
- Current CPR certification (BLS required)
- Board Certification via AANP or ANCC
- Minimum of 6 months nurse practitioner experience is preferred
- MSN degree from an accredited nursing school
