Play a vital role in improving patient care by performing comprehensive home health risk assessments for Medicare Advantage plan members across Fayette County, Ohio. This per diem opportunity is ideal for Nurse Practitioners seeking flexibility and meaningful work in a home health setting.
Schedule:
- Flexible shifts available Monday through Sunday

- Minimum weekly or bi-weekly hourly commitment, tailored to your preferences
Key Responsibilities:
- Conduct annual home health risk assessments in patient homes throughout Jeffersonville, Bloomingburg, Milledgeville, Washington Court House, and other areas within Fayette County
- Assess prior and current medical history, review existing medical conditions, and evaluate medication use
- Educate patients regarding their health conditions and medications, fostering better understanding and health management
- Identify gaps in care and make actionable recommendations to optimize patient outcomes
- No requirement to develop treatment plans or prescribe medications during these assessments
Qualifications:
- Active Nurse Practitioner license in Ohio
- Current CPR certification (BLS)
- Board certified through AANP or ANCC
- MSN degree from an accredited nursing program
- At least 6 months of experience as an NP preferred
Home health risk assessments generally last approximately 45 minutes and may include annual wellness checks or post-discharge visits. You will be compensated at $80$90 per evaluation, along with mileage reimbursement and compensation for administrative tasks, making this position both rewarding and worthwhile.
