Job Description

Work within Drew County, AR, serving Medicare Advantage plan members in communities such as Monticello, Wilmar, and Winchester. This role involves visiting patients in their homes to conduct comprehensive assessments, typically lasting around 45 minutes, which may include annual health evaluations or post-discharge visits. Build your own schedule with the convenience of shifts available between Monday and Sunday, and the option for minimum weekly or bi-weekly hourly commitments.

Key Responsibilities:

  • Conduct annual home health risk assessments for Medicare Advantage members.
  • Review and document prior/current medical history, existing conditions, and medication use.
  • Educate patients on their diagnoses and medications.
  • Identify and communicate potential gaps in care, recommending actionable next steps.
  • No requirement for treatment plan creation or medication prescribing.

Qualifications:

  • Active Nurse Practitioner License in AR.
  • Board Certification from AANP or ANCC.
  • Current CPR certification (BLS).
  • MSN from an accredited nursing program.
  • Minimum of 6 months NP experience preferred.

Compensation: Earn $80$90 per evaluation, in addition to a mileage stipend and compensation for administrative time.
