Job Description
Make a significant impact on patient well-being by performing home health risk assessments for Medicare Advantage plan members in Drew County, AR. This per diem Nurse Practitioner position offers flexible hours, allowing you to create your own schedule with shifts available seven days a week and customizable weekly or bi-weekly hour commitments.

Key Responsibilities:
- Perform comprehensive home health risk assessments in patient residences throughout Drew County, including Monticello, Wilmar, and Winchester
- Conduct annual assessments, post-discharge visits, and other varied evaluations (each typically lasting about 45 minutes)
- Review and evaluate patient medical histories, current health conditions, and medication use
- Educate members on identified medical conditions and proper medication management
- Detect care gaps and make recommendations to address these gaps
- No development of treatment plans or prescribing medications required
Qualifications:
- Current Nurse Practitioner license in Arkansas (AR)
- Board certification by AANP or ANCC
- MSN degree or higher from an accredited school of nursing
- Current CPR (BLS) certification
- At least 6 months of current NP experience is preferred
