Step into a dynamic per diem opportunity designed for Nurse Practitioners seeking flexible hours and varied patient interaction in the home health sector. In this role, youll conduct comprehensive risk assessments for Medicare Advantage plan members. Each home visit will average approximately 45 minutes and may include annual health assessments or post-discharge follow-ups. Scheduling is location-based, with coverage throughout El Paso County, including the Colorado Springs, Fountain, and Monument areas.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling options
- Minimum required weekly or bi-weekly hour commitment
Key Responsibilities:
- Complete thorough home health risk assessments for Medicare Advantage members
- Review and evaluate both previous and current medical history, ongoing conditions, and all medication regimens
- Offer education on medical conditions and prescribed medications to patients and their families
- Identify care gaps and recommend actionable solutions to improve patient outcomes
- Note: No prescription writing or treatment plan development is required in this role
Qualifications:
- Active Nurse Practitioner license in Colorado
- Current BLS CPR certification
- Board Certification through the AANP or ANCC
- At least 6 months of recent Nurse Practitioner experience is preferred
- Master of Science in Nursing (MSN) from an accredited institution
As part of this per diem assignment, you will receive $80$90 per evaluation, in addition to a mileage stipend and compensation for administrative tasks.
