Job Description
Deliver comprehensive care in the home setting as a Nurse Practitioner, specializing in home health risk assessments for Medicare Advantage plan members. This per diem opportunity is ideal for practitioners seeking flexibility and meaningful impact with each visit. Assignments are available across El Paso County, with patients residing in areas such as Fountain, Colorado Springs, and Monument. You will create your own schedule and may be assigned to different locations within the county during each shift.
Schedule:
- Flexible shifts offered Monday through Sunday
- Create your own schedule
- Minimum commitment required weekly or bi-weekly
Responsibilities:
- Conduct thorough home health risk assessments, averaging 45 minutes per visit
- Review previous and current medical history, conditions, and medication lists
- Educate members on their health conditions and medication management
- Identify gaps in care and recommend pathways for resolution
- Visits may include annual health assessments or post-discharge evaluations
- No need to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Colorado
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- Minimum of six months nurse practitioner experience preferred
- MSN degree from an accredited program
