Job Description

Step into a vital role where your expertise as a Nurse Practitioner will directly impact patient outcomes in the home health setting. Youll be responsible for conducting home health risk assessments for Medicare Advantage plan members throughout Allen County, and your work may include annual health assessments or post-discharge visits. Each in-home evaluation averages 45 minutes, and you will visit members residing in a variety of locations, including Fort Wayne, New Haven, and Huntertown.

Schedule:

  • Flexible shifts available seven days a week

  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Perform comprehensive in-home health risk assessments for Medicare Advantage members
  • Review and evaluate prior and current medical history, medical conditions, and medications
  • Educate members regarding their health conditions and medication usage
  • Identify and communicate any gaps in care, providing actionable recommendations to healthcare teams
  • Enjoy a focused role: no treatment planning or medication prescribing needed

Qualifications:

  • Active Nurse Practitioner license in Indiana
  • Board Certification through AANP or ANCC
  • Current CPR certification (BLS)
  • MSN degree from an accredited school of nursing
  • At least 6 months of recent NP experience preferred

This per diem opportunity offers compensation of $80-$90 per evaluation, plus a mileage stipend and pay for administrative time.
