Job Description
Deliver compassionate, patient-centered care in a per diem Nurse Practitioner position focused on home health risk assessments. This role involves traveling within Crawford County, ARincluding Van Buren, Alma, and Mulberryto perform evaluations for Medicare Advantage plan members. Each assessment generally takes about 45 minutes and may be an annual health checkup or a post-discharge review.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-schedulingdesign your own workweeks
- Opportunities for minimum weekly or bi-weekly commitments
Key Responsibilities:
- Complete on-site home health risk assessments for Medicare Advantage plan members
- Assess and document prior and current medical histories, ongoing conditions, and medication usage
- Educate patients about their medical conditions and medications to optimize their health understanding
- Identify care gaps and recommend next steps for resolution
- No requirement to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner License in Arkansas
- MSN degree from an accredited nursing school
- Board Certification through AANP or ANCC
- Current CPR certification (BLS)
- At least 6 months of recent Nurse Practitioner experience preferred
