This per diem position centers around conducting risk assessments for Medicare Advantage members throughout Crawford County, AR, including the areas of Van Buren, Alma, and Mulberry. You will meet patients in their homes and deliver high-quality, compassionate care tailored to their unique needs.
Schedule:
- Flexible shifts available any day, Monday through Sunday
- Design your own work schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage plan members
- Evaluate patients medical histories, existing conditions, and medication regimens
- Educate patients about their medical conditions and medications
- Identify and address gaps in care; provide clear recommendations for improvement
- No involvement in developing treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner License in Arkansas
- Current CPR/BLS certification
- Board Certification (AANP or ANCC)
- Completion of an MSN from an accredited nursing program
- Minimum of 6 months experience as a Nurse Practitioner (preferred)
This position offers pay of $80$90 per evaluation, with a mileage stipend and compensation for administrative activities.
