Enhance patient outcomes by delivering personalized home health risk assessments for Medicare Advantage plan members in Fulton County, NY. This per diem opportunity is tailored for an experienced Nurse Practitioner seeking flexibility, autonomy, and the ability to make a meaningful impact within the community.

Schedule:

Key Responsibilities:

  • Perform comprehensive home health risk assessments in patient residences
  • Assess medical history, ongoing conditions, and medication usage
  • Deliver patient education about diagnoses and medications
  • Identify potential gaps in care; recommend appropriate next steps
  • Visit durations average 45 minutes per assessment
  • No requirement to write treatment plans or prescribe medications

Qualifications:

  • Valid Nurse Practitioner License in New York
  • MSN degree from an accredited nursing school
  • Current CPR (BLS) certification
  • Board Certification through AANP or ANCC
  • At least 6 months of recent NP clinical experience preferred
