This role involves visiting Medicare Advantage plan members throughout Fulton County, including communities such as Gloversville, Johnstown, and Broadalbin. Flexible scheduling allows you to create your own shifts between Monday and Sunday, with the option to choose weekly or bi-weekly hourly commitmentsperfect for balancing other clinical responsibilities or personal priorities.
Key Responsibilities:
- Conduct comprehensive annual health and post-discharge risk assessments for Medicare Advantage members in their homes.
- Review and evaluate patients prior and current medical history, conditions, and medication use.
- Deliver education to patients about their health conditions and prescribed medications.
- Identify care gaps and offer recommendations to improve patient outcomes.
- No requirement for creating ongoing treatment plans or prescribing medication.

Qualifications:
- Active Nurse Practitioner license in New York State
- Board Certification (AANP or ANCC)
- MSN from an accredited nursing school
- Current CPR (BLS) certification
- At least 6 months of Nurse Practitioner experience preferred
