As a skilled Nurse Practitioner specializing in home health, you'll perform comprehensive risk assessments for Medicare Advantage plan members across Lee County, FLincluding Cape Coral, Fort Myers, and Lehigh Acres. The role allows you to create your own schedule with minimum weekly or bi-weekly hourly commitments, supporting a healthy work-life balance.

Key Responsibilities:
- Conduct annual home health risk assessments for members of a Medicare Advantage health plan, taking approximately 45 minutes per visit
- Review prior and current medical histories, health conditions, and medication use
- Educate patients on their chronic and acute medical conditions and medications
- Identify gaps in care and suggest resolutions to improve patient outcomes
- Visits do not involve creating treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in Florida
- CPR certification (BLS) required
- Board Certification through AANP or ANCC
- Minimum of 6 months experience as a Nurse Practitioner preferred
- Master of Science in Nursing (MSN) from an accredited program
