Deliver valuable healthcare services directly to patients homes as a skilled Nurse Practitioner focused on Home Health risk assessments. This per diem opportunity is ideal for those seeking meaningful patient engagement and flexible scheduling in Lee County, including Cape Coral, Fort Myers, and Lehigh Acres.
Schedule Highlights:
- Flexible shifts available Monday through Sunday

- Commit to a minimum number of hours weekly or bi-weekly based on your availability
Key Responsibilities:
- Conduct comprehensive annual and post-discharge risk assessments for Medicare Advantage members in their homes
- Review and evaluate patient medical histories, including previous and current conditions and medications
- Educate patients about their health status and prescribed medications
- Identify care gaps and offer actionable recommendations to support patient wellness
- Please note: No direct treatment planning or prescribing is required
Required Qualifications:
- Active Nurse Practitioner license in Florida
- Board certification through AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited program
- Current CPR certification (BLS)
- At least 6 months of recent experience as an NP is highly preferred
