Job Description

Seeking an experienced Nurse Practitioner to perform home health risk assessments on a per diem basis. This opportunity focuses on providing high-quality in-home evaluations for Medicare Advantage plan members across Broward County, including Fort Lauderdale, Pembroke Pines, and Hollywood. Each risk assessment typically takes 45 minutes and may involve annual health assessments or post-discharge visits. The work environment supports autonomy and enables you to manage your own flexible schedule.

Schedule:

  • Flexible shifts available between Monday and Sunday
  • Ability to create your own schedule
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Perform comprehensive home health risk assessments for Medicare Advantage plan members
  • Review prior and current medical history, conditions, and medication regimens
  • Deliver patient education on medical conditions and prescribed medications
  • Identify care gaps and provide actionable recommendations to close those gaps
  • No responsibility for developing treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in Florida
  • Current CPR (BLS) certification
  • Board Certification through AANP or ANCC
  • Six months or more of current Nurse Practitioner experience is preferred
  • MSN degree from an accredited nursing program

This role offers competitive pay, with $80-$90 per evaluation, along with a mileage stipend and compensation for administrative time.
