Deliver valuable care in the homes of Medicare Advantage plan members by conducting comprehensive health risk assessments. This per diem opportunity offers exceptional flexibility, with the ability to create your own schedule and the freedom to select shifts between Monday and Sunday. As a Nurse Practitioner specializing in home health, you'll focus on locations throughout Muskogee County, including Fort Gibson, Muskogee, and Warner. Each assessment typically takes 45 minutes and may include annual wellness evaluations or post-discharge visits.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own preferred schedule
- Minimum hourly commitment required weekly or bi-weekly
Key Responsibilities:
- Perform in-depth annual home health risk assessments for Medicare Advantage plan members
- Assess both current and previous medical histories, ongoing medical conditions, and medication usage
- Educate members on their medical conditions and medication purposes/effects
- Recognize potential gaps in care and suggest actionable solutions
- No need to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner licensure in Oklahoma
- Valid CPR certification (BLS required)
- Board certification through AANP or ANCC
- Minimum of 6 months current NP experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
This role offers competitive per evaluation compensation between $80-$90, supplemented by a mileage stipend and additional compensation for administrative time.
