Deliver comprehensive care as a Nurse Practitioner specializing in home health risk assessments within Muskogee County. This per diem opportunity allows you to create your own schedule with flexible shifts available any day of the week, Monday through Sunday. Enjoy autonomy and variety by working in diverse locations throughout Muskogee County, including Muskogee, Fort Gibson, and Warner.
Schedule:
- Flexible shifts Monday through Sunday
- Set your own hours
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage plan members in their homes
- Assess and document medical history, medical conditions, and medication use
- Provide patient education regarding existing medical conditions and medications
- Identify potential gaps in care and recommend actions to address them
- Visits may range from annual assessments to post-discharge check-ins
- No need to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Oklahoma
- Current CPR certification (BLS)
- Board Certification as a Nurse Practitioner (AANP or ANCC)
- Minimum of 6 months experience as an NP preferred
- MSN from an accredited nursing program
Compensation for this role is provided per completed evaluation, with additional mileage and administrative stipends included. Earn between $80 and $90 for each risk assessment completed, ensuring your efforts are recognized and rewarded in a flexible, supportive environment.
