Job Description

Deliver impactful care by performing home health risk assessments for Medicare Advantage plan members throughout Jefferson County. In this per diem position, Nurse Practitioners use their clinical expertise to enhance patient outcomes in the comfort of each patient's home. Assessments typically last about 45 minutes and may include annual health reviews or post-discharge visits. Flexible scheduling options allow you to create your own workweek, with shifts available Monday through Sunday. You may be scheduled for visits in Le Ray, Watertown, Adams, Fort Drum, or other neighboring communities during your shifts.

Schedule:

  • Flexible shifts between Monday and Sunday

  • Minimum weekly or bi-weekly hourly commitment

Responsibilities:

  • Complete comprehensive home health risk assessments for Medicare Advantage members
  • Review past and current medical history, health conditions, and medications
  • Educate patients regarding their diagnoses and medications
  • Identify care gaps and make recommendations to close them
  • Strictly assessment-focusedno prescribing or developing treatment plans required

Qualifications:

  • Active Nurse Practitioner license in NY
  • Board certification through AANP or ANCC
  • Current BLS (CPR) certification
  • MSN from an accredited nursing program
  • At least 6 months of recent NP experience preferred
