Deliver meaningful care and make a difference in patients lives by providing home health risk assessments as a Nurse Practitioner working per diem. Youll play an integral role in helping Medicare Advantage members maintain their health and well-being through comprehensive assessments completed in the comfort of their homes.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Commit to a minimum number of hours weekly or bi-weekly
Key Responsibilities:
- Complete annual home health risk assessments with Medicare Advantage plan members
- Evaluate both past and current medical history, including medical conditions and medications
- Offer education to patients regarding their health conditions and prescribed medications
- Spot gaps in care and offer recommendations to support patients overall health
- No treatment plan creation or medication prescribing required
Qualifications:
- Active Nurse Practitioner license in Colorado
- Current Basic Life Support (CPR) certification
- Board Certification through AANP or ANCC
- MSN degree from an accredited school of nursing
- At least 6 months of current NP experience preferred
