Job Description

This per diem Nurse Practitioner opportunity offers a rewarding way to make a meaningful impact in the community by providing in-home health risk assessments for Medicare Advantage members. You'll enjoy true flexibility while connecting with patients across Cumberland County, including areas such as Fayetteville, Hope Mills, Spring Lake, and Eastover. Home health visits may include annual health assessments and post-discharge check-ins, with each taking approximately 45 minutes.

Schedule:

  • Flexible shifts available seven days a week
  • Choose your own workdays and hours
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Perform annual risk assessments and post-discharge visits for Medicare Advantage plan members in their homes
  • Review and evaluate prior and current medical history, conditions, and medication regimens
  • Deliver patient education about ongoing medical issues and prescribed medications
  • Identify care gaps and recommend potential solutions and follow-up actions
  • Document all findings; no prescribing or creation of treatment plans required

Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • Board certification through AANP or ANCC
  • Current BLS certification
  • MSN from an accredited nursing program
  • At least 6 months of recent Nurse Practitioner experience (preferred)
  • Strong communication and assessment skills
