Enjoy the flexibility of a per diem role as a Nurse Practitioner providing essential home health risk assessments to Medicare Advantage plan members in St. Francis County. Schedule:
- Flexible shifts available Monday through Sunday
- Choose your own working days and hours
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform comprehensive home health risk assessments, each lasting approximately 45 minutes, for Medicare Advantage members across St. Francis County, including Forrest City, Hughes, and Madison.
- Evaluate medical histories, current medical conditions, and medication use during visits.
- Educate individuals on their health conditions and medications.
- Identify care gaps and make recommendations for improvement, supporting better health outcomes.
- No responsibility for creating treatment plans or prescribing medicationsfocus exclusively on assessment and education.
Qualifications:
- Active Nurse Practitioner license in Arkansas
- Current BLS or CPR certification
- Board certification through AANP or ANCC
- At least 6 months of recent NP experience preferred
- MSN degree from an accredited nursing program
