Job Description

Make a meaningful impact in healthcare delivery by providing home health risk assessments to Medicare Advantage plan members throughout St. Francis County, AR. This per diem opportunity offers the flexibility to design your schedule, working shifts that fit your lifestyle, with assessments conducted in locations such as Forrest City, Hughes, and Madison.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Set your own working hours
  • Minimum weekly or bi-weekly hourly commitment based on your availability

Key Responsibilities:

  • Perform comprehensive annual and post-discharge home health risk assessments for Medicare Advantage members
  • Thoroughly review and document patients medical history, current conditions, and medication usage
  • Offer patient education regarding health conditions and prescribed medications
  • Identify care gaps and share recommendations to support high-quality care outcomes
  • Focus exclusively on patient assessmentsprescribing medications and devising treatment plans are not part of this role

Qualifications:

  • Active Nurse Practitioner license in Arkansas
  • Current CPR certification (BLS)
  • Board Certification via AANP or ANCC
  • Minimum of 6 months Nurse Practitioner experience preferred
  • Master of Science in Nursing (MSN) from an accredited institution

This role is compensated at $80-$90 per evaluation, with additional mileage reimbursement and pay for administrative duties, reflecting the value placed on your expertise and time.
