Advance patient care directly in the comfort of patients homes by conducting thorough health risk assessments for Medicare Advantage plan members. This per diem opportunity involves traveling to various locations within Pike County, including Glenwood, Murfreesboro, and Delight. With flexible scheduling and the ability to create your own hours, this role is ideal for Nurse Practitioners seeking autonomy and a meaningful impact on patient outcomes.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-schedule your shifts
  • Minimum hourly commitment on a weekly or bi-weekly basis

Key Responsibilities:

  • Perform in-home annual health risk assessments for Medicare Advantage plan members
  • Review and evaluate medical history, current conditions, and medication usage
  • Educate patients on their health conditions and prescribed medications
  • Detect care gaps and offer recommendations for improved outcomes
  • Complete documentation efficiently; treatment planning and prescribing are not required

Qualifications:

  • Active Nurse Practitioner license in Arkansas
  • Board Certification through AANP or ANCC
  • CPR certification (BLS) current and in good standing
  • MSN from an accredited school of nursing
  • At least 6 months of current NP experience preferred
