- Set your preferred minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct annual and post-discharge home health risk assessments for Medicare Advantage members in assigned Lee County locations
- Review patients medical histories, chronic and acute medical conditions, and current medications during each 45-minute assessment
- Educate patients on disease management, prevention, and safe medication use
- Identify and report care gaps, providing actionable recommendations to improve patient health outcomes
- No requirement to write treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Arkansas
- Current CPR certification (BLS)
- National Board Certification through AANP or ANCC
- At least 6 months of recent Nurse Practitioner experience preferred
- MSN degree from an accredited nursing program
