Job Description
Deliver high-quality care directly to patients through home health risk assessments as a Nurse Practitioner within Lee County, AR, including Marianna, Moro, and Rondo. This per diem opportunity enables you to visit Medicare Advantage plan members in their homes, performing comprehensive health evaluations focused on improving patient outcomes and supporting preventive care. Each assessment typically lasts about 45 minutes and could take the form of an annual review or post-discharge check-in.

Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage members
- Assess medical history, current conditions, and medication usage
- Educate patients about their health conditions and medications
- Identify care gaps and recommend next steps for resolution
- No requirement to develop treatment plans or prescribe medications
Preferred Qualifications:
- Nurse Practitioner license in Arkansas
- Board certification (AANP or ANCC)
- MSN from an accredited nursing program
- Current CPR (BLS) certification
- Minimum 6 months of NP experience preferred
