Schedule:
- Create your own schedule
- Flexible shifts available Monday through Sunday
- Set minimum weekly or bi-weekly hourly commitments to fit your lifestyle
Key Responsibilities:
- Conduct annual home health risk assessments for Medicare Advantage members, averaging 45 minutes per visit
- Evaluate prior and current medical history, existing conditions, and medication history
- Educate members about their health conditions and medications
- Identify care gaps and provide recommendations to address them
- Focus solely on assessing and recommending; no need to prescribe medication or develop treatment plans
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Current CPR certification (BLS)
- Board Certified through AANP or ANCC
- MSN degree from an accredited nursing program
- At least 6 months of recent Nurse Practitioner experience preferred
