Advance your career as a Nurse Practitioner specializing in home health services. This per diem opportunity involves conducting in-depth home health risk assessments for Medicare Advantage plan members throughout Dallas County, AR, including areas such as Fordyce, Sparkman, and Carthage. You will play a vital role in ensuring members receive quality, comprehensive evaluations from the comfort of their own homes.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule to fit your availability
- Minimal required weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete annual home health risk assessments, focusing on thorough reviews of members prior and current medical history, existing conditions, and medication lists
- Deliver education to patients regarding their health conditions and medication regimens
- Identify gaps in care and provide clear recommendations for follow-up
- Conduct assessments onlyno management of treatment plans or prescription-writing involved
- Average assessment duration is approximately 45 minutes
Qualifications:
- Active Nurse Practitioner license in Arkansas
- Current CPR (BLS) certification
- Board Certification through AANP or ANCC
- Minimum of six months recent Nurse Practitioner experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
