Job Description

This per diem opportunity involves conducting in-depth home health risk assessments for Medicare Advantage plan members throughout Dallas County, AR, including areas such as Fordyce, Sparkman, and Carthage. You will play a vital role in ensuring members receive quality, comprehensive evaluations from the comfort of their own homes.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule to fit your availability
  • Minimal required weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Complete annual home health risk assessments, focusing on thorough reviews of members prior and current medical history, existing conditions, and medication lists
  • Deliver education to patients regarding their health conditions and medication regimens
  • Identify gaps in care and provide clear recommendations for follow-up
  • Conduct assessments onlyno management of treatment plans or prescription-writing involved
  • Average assessment duration is approximately 45 minutes

Qualifications:

  • Active Nurse Practitioner license in Arkansas
  • Current CPR (BLS) certification
  • Board Certification through AANP or ANCC
  • Minimum of six months recent Nurse Practitioner experience preferred
  • Master of Science in Nursing (MSN) from an accredited institution
