Job Description
Deliver vital healthcare directly to patients in the comfort of their homes as a Nurse Practitioner providing home health risk assessments. This per diem position offers the flexibility to create your own schedule, with shifts available between Monday and Sunday. Youll work in dynamic environments, supporting Medicare Advantage plan members by performing thorough annual assessments or post-discharge visits.
Schedule:
- Flexible shifts: Monday to Sunday
- Choose your own working hours
- Commitment can be weekly or bi-weekly, based on preference
Key Responsibilities:
- Conduct comprehensive home health risk evaluations for Medicare Advantage members
- Assess medical history, existing conditions, and current medication regimens
- Educate patients about their medical conditions and prescribed medications
- Identify care gaps and provide actionable recommendations to improve patient outcomes
- Focus on assessment; treatment planning and medication prescribing are not required
Qualifications:
- Active Nurse Practitioner license, valid in NY
- Board certification through AANP or ANCC
- Current BLS/CPR certification
- MSN degree from an accredited nursing program
- At least 6 months of NP experience preferred
