Seeking a skilled Nurse Practitioner to perform home health risk assessments for Medicare Advantage plan members throughout Westchester County, NY, including areas such as Yonkers, New Rochelle, Mount Vernon, and White Plains. This per diem position offers the flexibility to customize your schedule and select shifts, with home visits averaging 45 minutes each. Duties primarily involve performing annual health assessments or post-discharge evaluations for members in their homes.

Schedule:

• Flexible shifts, Monday through Sunday
• Self-schedulingset your own hours
• Minimum weekly or bi-weekly hourly commitment

Responsibilities:

• Complete annual home health risk assessments for Medicare Advantage members
• Review and evaluate current and prior medical history, conditions, and medications
• Educate patients on managing their health and understanding their medication regimens
• Identify care gaps and recommend steps for improvement to the member and their health plan
• No prescribing or treatment plan generation required

Qualifications:

• Active Nurse Practitioner license in New York State
• MSN degree from an accredited nursing program
• Board certification through AANP or ANCC
• Current CPR (BLS) certification required
• At least 6 months of recent NP experience preferred

This role offers competitive earnings of $80-$90 per evaluation, along with a mileage stipend and compensation for administrative time.
