Job Description

Make a difference in the lives of Medicare Advantage plan members across Westchester County, conducting comprehensive in-home health assessments and empowering patients with knowledge about their conditions and medication needs.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Set your own schedule based on personal availability
  • Weekly or bi-weekly minimum hourly commitment

Key Responsibilities:

  • Conduct annual, post-discharge, or other home health risk assessments for Medicare Advantage health plan members residing in areas such as Yonkers, New Rochelle, Mount Vernon, and White Plains
  • Review and evaluate medical histories, chronic and acute conditions, and medication use with each member
  • Educate patients on their health status and outline important medication and self-care information
  • Identify gaps in care and recommend steps for improved healthcare managementno need to develop treatment plans or prescribe medications

Required Qualifications:

  • Nurse Practitioner License in New York
  • MSN degree from an accredited nursing program
  • Board Certification via AANP or ANCC
  • Current CPR certification (BLS)
  • At least 6 months of recent Nurse Practitioner experience preferred, especially in home health
