This per diem opportunity focuses on conducting comprehensive risk evaluations for Medicare Advantage members throughout Fremont County, ensuring high standards of patient care in the comfort of patients homes.

Schedule:

• Flexible shifts available Monday through Sunday
• Design your own work schedule to best suit your lifestyle
• Minimum commitment per week or every two weeks

Key Responsibilities:

• Complete annual health and post-discharge risk assessments for Medicare Advantage members in various Fremont County communities
• Review and assess prior and current medical history, health conditions, and medication regimens
• Deliver patient education on conditions and medications to enhance understanding and compliance
• Detect gaps in care and clearly recommend follow-up actions
• Enjoy a focused assessment role, with no requirement for creating treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner license in the state of Colorado
• Current CPR certification (BLS)
• National board certification (AANP or ANCC)
• At least six months of current nurse practitioner experience preferred
• Master of Science in Nursing (MSN) from an accredited institution
