Take on a rewarding per diem opportunity providing home health risk assessments for Medicare Advantage plan members across Cass County, IN. As a Nurse Practitioner in this essential role, youll provide crucial evaluations in residents homes throughout areas such as Logansport, Galveston, and Walton. Youll make a significant impact by ensuring continuity of care for members following hospital discharge or as part of their annual health review.
Schedule:
- Flexible shifts available seven days a week
- Design your own schedule to fit your availability
- Minimum hourly commitment by week or bi-weekly
Key Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage patients
- Assess prior and current medical history, review chronic conditions, and evaluate medication usage
- Offer patient education on chronic illnesses and prescribed medications
- Spot gaps in care and propose recommendations to improve patient health outcomes
- No need to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner License in Indiana
- Current BLS (CPR) certification
- Board Certified through AANP or ANCC
- MSN from an accredited nursing program
- Minimum six months of NP experience is desirable
