Schedule:
- Flexible shifts available seven days a week (MondaySunday)

- Minimum required weekly or bi-weekly hours
Role Overview:
- Complete comprehensive home health risk assessments for Medicare Advantage plan participants in various settings, including annual wellness visits and post-discharge follow-ups
- Assess patients prior and current medical histories, ongoing conditions, and medication regimens
- Deliver patient-centered education on chronic conditions and individualized medication instructions
- Detect gaps in care and recommend effective solutions to improve patient health outcomes
- There is no responsibility for developing treatment plans or prescribing medications; your focus is evaluation and education
Desired Qualifications:
- Current Nurse Practitioner license in the state of Indiana
- Active Basic Life Support (BLS/CPR) certification
- Board certified through AANP or ANCC
- At least six months of Nurse Practitioner experience preferred
- MSN degree from an accredited institution
