Job Description

Schedule:

  • Flexible shifts available seven days a week (MondaySunday)

  • Minimum required weekly or bi-weekly hours

Role Overview:

  • Complete comprehensive home health risk assessments for Medicare Advantage plan participants in various settings, including annual wellness visits and post-discharge follow-ups
  • Assess patients prior and current medical histories, ongoing conditions, and medication regimens
  • Deliver patient-centered education on chronic conditions and individualized medication instructions
  • Detect gaps in care and recommend effective solutions to improve patient health outcomes
  • There is no responsibility for developing treatment plans or prescribing medications; your focus is evaluation and education

Desired Qualifications:

  • Current Nurse Practitioner license in the state of Indiana
  • Active Basic Life Support (BLS/CPR) certification
  • Board certified through AANP or ANCC
  • At least six months of Nurse Practitioner experience preferred
  • MSN degree from an accredited institution
