Seeking a skilled Nurse Practitioner to conduct home health risk assessments within Genesee County. In this per diem role, youll complete comprehensive in-home evaluations for Medicare Advantage members, visiting patients in Flint, Grand Blanc, Burton, and nearby areas. Each assessment typically takes around 45 minutes and may include annual wellness checks or post-discharge visits. The schedule offers significant flexibility, allowing you to set your own hours and choose shifts between Monday and Sunday, with weekly or bi-weekly hourly commitments.

Key Responsibilities:

• Perform in-home annual risk assessments for Medicare Advantage members
• Review and document patients prior and current medical history, conditions, and medications
• Educate patients on managing medical conditions and medication adherence
• Identify gaps in care and recommend steps for resolution
• Focus solely on assessmentsno treatment plans or prescribing required

Qualifications:

• Active Nurse Practitioner (NP) license in Michigan
• Board certification via AANP or ANCC
• MSN from an accredited nursing program
• Current Basic Life Support (BLS/CPR) certification
• At least 6 months of recent NP experience preferred
