Job Description

Deliver compassionate, high-quality care as a Nurse Practitioner specializing in home health risk assessments across Genesee County. This per diem opportunity gives you the autonomy to design your own schedule, choosing flexible shifts Monday through Sunday with the potential to work in diverse areas such as Flint, Grand Blanc, and Burton.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Choose your own working days and times
  • Minimum weekly or bi-weekly hourly commitment

Primary Responsibilities:

  • Perform annual risk assessments for Medicare Advantage plan members in their homes
  • Review and evaluate medical histories, ongoing conditions, and medication lists
  • Offer education to members about their health conditions and current medications
  • Spot gaps in care and recommend steps to address them
  • No responsibility for prescribing medications or developing treatment plans

Qualifications:

  • Current, active Nurse Practitioner licensure in Michigan
  • Maintains CPR/BLS certification
  • Board certified through AANP or ANCC
  • Holds an MSN degree from an accredited nursing school
  • At least 6 months of recent Nurse Practitioner experience preferred

This role comes with competitive compensation of $80$90 per evaluation, along with a mileage stipend and additional pay for administrative time.
