Step into a meaningful per diem opportunity as a Nurse Practitioner providing home health risk assessments in the Genesee County area. You will be visiting Medicare Advantage plan members in their homes to conduct annual health assessments, post-discharge visits, and other wellness evaluations. Each visit typically takes about 45 minutes and focuses on comprehensive health reviewsno treatment planning or medication prescribing required.

Schedule/Hours

• Flexible scheduling available Monday through Sunday
• Ability to set your own hours and frequency of shifts
• Minimum required commitment on a weekly or bi-weekly basis

Key Responsibilities

• Perform thorough home health risk assessments for Medicare Advantage members
• Review prior and current medical history, existing conditions, and medication usage
• Educate members about their health status, chronic conditions, and medication management
• Identify gaps in care and recommend next steps for primary care follow-up
• Complete accurate documentation in a timely manner

Qualifications

• Active Nurse Practitioner license in Michigan
• Board Certification (AANP or ANCC)
• MSN from an accredited nursing program
• Current CPR certification (BLS)
• At least 6 months of recent experience as an NP is preferred
• Strong interpersonal and communication skills for working directly with patients in their homes

This position offers compensation of $80$90 per completed evaluation, plus a mileage stipend and administrative time reimbursement.
