This role involves traveling within Henderson County, where home visits to assess Medicare Advantage plan members are essential. Each assessment typically takes around 45 minutes and may vary from annual health evaluations to post-discharge check-ins.

Schedule:

• Flexible shifts available Monday through Sunday
• Set your own work schedule
• Minimum hourly commitment on a weekly or bi-weekly basis

Responsibilities:

• Conduct comprehensive home health risk assessments on Medicare Advantage plan members residing throughout Henderson County, including Hendersonville, Fletcher, and Mills River
• Review and evaluate patients medical history, current conditions, and medication regimens
• Provide clear education to patients about their conditions and medications
• Identify gaps in care and recommend steps for improvement when applicable
• No requirement to create treatment plans or prescribe medications

Qualifications:

• Active North Carolina Nurse Practitioner license
• Board Certification through AANP or ANCC
• MSN degree from an accredited school of nursing
• Current CPR certification (BLS)
• At least 6 months of NP experience strongly preferred
