Job Description
Deliver compassionate care in the comfort of patients homes as a Nurse Practitioner specializing in home health risk assessments. This per diem position offers the flexibility to design your own schedule, working shifts that fit your life between Monday and Sunday, across locations throughout Duval County. Assessments typically take 45 minutes and focus on Medicare Advantage plan members in the area. Each shift may include travel to various communities within Duval County, providing a broad range of impactful encounters.
Schedule:
- Choose your shiftswork any days Monday through Sunday
- Set your own hours, with a minimum weekly or bi-weekly commitment
Key Responsibilities:
- Conduct comprehensive annual risk assessments during home visits with Medicare Advantage plan members
- Collect and review prior and current medical history, existing conditions, and medication usage
- Educate patients about their diagnoses and prescribed treatments
- Identify gaps in care and recommend actionable next steps
- No prescription of medications or development of treatment plans required
Qualifications:
- Active Nurse Practitioner license in Florida
- Board certification from AANP or ANCC
- MSN degree from an accredited nursing school
- Current CPR (BLS) certification
- At least 6 months of recent NP experience preferred
