Enjoy a flexible, rewarding per diem opportunity performing home health risk assessments as a Nurse Practitioner across Flagler County. This role offers the freedom to create your own schedule and supports clients throughout the region, including Palm Coast, Flagler Beach, and Bunnell. Each assessment takes approximately 45 minutes and may involve annual health check-ups or post-discharge visits for Medicare Advantage plan members.
Schedule:
- Flexible shifts available between Monday and Sunday
- Ability to design your own schedule
- Choose a weekly or bi-weekly minimum hourly commitment to fit your availability
Primary Responsibilities:
- Conduct comprehensive annual home health risk assessments for Medicare Advantage members
- Review prior and current medical history, medication regimens, and overall health conditions
- Educate patients on health concerns and proper medication management
- Identify gaps in care, offering actionable recommendations to improve patient outcomes
- Focus exclusively on patient evaluationsno requirement to prescribe medications or develop treatment plans
Qualifications:
- Current Nurse Practitioner license in Florida
- Active CPR (BLS) certification
- Board certification through AANP or ANCC
- MSN degree from an accredited nursing institution
- Minimum of 6 months experience as a Nurse Practitioner preferred
This role provides compensation of $80-$90 per evaluation, plus a mileage stipend and additional compensation for administrative time.
