Job Description
Deliver high-quality care and guidance as a Nurse Practitioner in a per diem home health setting, focusing on comprehensive risk assessments for Medicare Advantage members. This opportunity provides autonomy and flexibility, allowing you to create your own schedule while supporting individuals throughout Hancock County.
Schedule:
- Flexible shifts are available every day of the week (Monday through Sunday).
- Choose your own working hours to suit your lifestyle.
- Commitment can be fulfilled on a weekly or bi-weekly basis as preferred.
Key Responsibilities:
- Perform thorough annual home health risk assessments for plan members in various Hancock County areas, including Greenfield, McCordsville, and Fortville.
- Assess patients prior and current medical history, chronic conditions, and medication regimens.
- Offer clear, relevant education regarding medical conditions and medications.
- Identify any care gaps and suggest resolutions to optimize ongoing health management.
- Visits may include annual assessments or follow-ups following hospital discharge.
- No responsibility for designing treatment plans or prescribing medications allows you to focus on clinical evaluation and health advocacy.
Qualifications:
- Active Nurse Practitioner license in Indiana.
- Board Certification through AANP or ANCC.
- MSN from an accredited school of nursing.
- Current BLS certification.
- At least six months of experience as an NP is preferred.
Compensation includes $80-$90 per completed evaluation, with an additional stipend provided for mileage and administrative time.
