Job Description

Advance your career as a Nurse Practitioner providing home health risk assessments on a flexible, per diem basis. In this dynamic role, youll serve adults enrolled in Medicare Advantage plans throughout Hamilton County, INincluding Carmel, Fishers, and Noblesville. Each visit is focused, with most assessments completed in approximately 45 minutes, and could include annual health evaluations or post-discharge visits.

Schedule:

  • Choose your own shifts with full flexibility, Monday through Sunday
  • Minimum hourly commitment, weekly or bi-weekly
  • Per diem structure allows you to plan around your life

Key Responsibilities:

  • Perform annual home health risk assessments for Medicare Advantage members
  • Assess prior and current medical history, active conditions, and medication history
  • Educate patients about their diagnoses and medication regimens
  • Detect gaps in care and recommend proactive measures, enhancing patient health outcomes
  • Focus solely on assessment and educationno prescribing or treatment planning required

Qualifications:

  • Active Nurse Practitioner license in Indiana
  • Current CPR certification (BLS)
  • Board certification through AANP or ANCC
  • 6 months of recent Nurse Practitioner experience preferred
  • Master of Science in Nursing from an accredited institution
