Job Description
Experienced nurse practitioners are needed for home health risk assessment visits for Medicare Advantage members throughout Bingham County. This per diem opportunity offers flexibility and autonomy, allowing practitioners to set their preferred schedules and locations within the service area.
Schedule/Hours
- Flexible shifts offered Monday through Sunday
- Self-scheduling availablecontrol your own shifts
- Minimum weekly or bi-weekly hourly commitments based on your availability
Responsibilities
- Perform annual home health risk assessments within members homes
- Review and evaluate patients medical histories, ongoing medical conditions, and medications
- Educate members on their health status, chronic conditions, and prescribed medications
- Detect gaps in care and suggest actionable recommendations to close those gaps
- Visits may include annual health assessments or post-discharge follow-ups
- No requirement for creating treatment plans or prescribing medications
Qualifications
- Active Nurse Practitioner license in Idaho
- Board Certification (AANP or ANCC)
- Current BLS/CPR certification
- Master of Science in Nursing (MSN) from an accredited institution
- At least 6 months of recent experience as an NP, preferably in home health settings
