Job Description

Experienced Nurse Practitioners are needed for per diem home health risk assessments within Bingham County. This flexible opportunity allows you to leverage your clinical skills while making a difference in your community. Assessments are conducted for Medicare Advantage members in residential locations throughout Blackfoot, Shelley, Aberdeen, and other areas in Bingham County.

Schedule:

  • Flexible shifts available between Monday and Sunday
  • Self-scheduled create a work routine that suits your lifestyle
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Perform comprehensive annual home health risk assessments for Medicare Advantage members
  • Review and evaluate both previous and current medical history, chronic conditions, and medication use
  • Educate patients on their health conditions, treatment recommendations, and medication instructions
  • Recognize care gaps and suggest resolutions to help improve overall patient outcomes
  • No responsibility for writing treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in Idaho required
  • Current CPR certification (BLS)
  • Board certification through AANP or ANCC
  • Master's degree (MSN) from an accredited nursing program
  • At least 6 months of experience as a Nurse Practitioner preferred
