Experienced Nurse Practitioners are needed for per diem home health risk assessments within Bingham County. This flexible opportunity allows you to leverage your clinical skills while making a difference in your community. Assessments are conducted for Medicare Advantage members in residential locations throughout Blackfoot, Shelley, Aberdeen, and other areas in Bingham County.
Schedule:
- Flexible shifts available between Monday and Sunday
- Self-scheduled create a work routine that suits your lifestyle
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Perform comprehensive annual home health risk assessments for Medicare Advantage members
- Review and evaluate both previous and current medical history, chronic conditions, and medication use
- Educate patients on their health conditions, treatment recommendations, and medication instructions
- Recognize care gaps and suggest resolutions to help improve overall patient outcomes
- No responsibility for writing treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in Idaho required
- Current CPR certification (BLS)
- Board certification through AANP or ANCC
- Master's degree (MSN) from an accredited nursing program
- At least 6 months of experience as a Nurse Practitioner preferred
