Seeking an experienced and compassionate Nurse Practitioner to provide home health risk assessments on a per diem basis throughout Genesee County, MI. In this role, you will conduct comprehensive health assessments for Medicare Advantage members, helping identify care needs and contributing to improved patient outcomes. This position offers flexibility and the chance to make a meaningful difference in patients lives outside of traditional clinical settings.

Schedule:

• Flexible shifts available Monday through Sunday
• Design your own schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform annual and post-discharge home health risk assessments for Medicare Advantage plan members residing in Flint, Grand Blanc, Burton, and surrounding areas of Genesee County
• Assess patients prior and current medical history, existing conditions, and medication histories
• Educate patients on their medical conditions and prescribed medications
• Identify care gaps and provide actionable recommendations to support care coordination
• No requirement for making treatment plans or prescribing medications

Required Qualifications:

• Current Nurse Practitioner license in Michigan
• Board Certification through AANP or ANCC
• Active CPR certification (BLS)
• Minimum of 6 months current NP experience preferred
• MSN degree from an accredited nursing school

Compensation for this opportunity is $80-$90 per completed evaluation, along with a mileage stipend and allowances for administrative time.
