This role serves Medicare Advantage members across Genesee County, ensuring high-quality, patient-centered care is delivered directly to their homes. Schedule:
- Flexible shifts available any day, Monday through Sunday
- Create your own schedule for optimal work-life balance
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform comprehensive annual and post-discharge home health risk assessments for Medicare Advantage members
- Review and evaluate patients' prior and current medical history, conditions, and medication use
- Provide targeted education on medical conditions and medication regimens
- Identify potential gaps in care and make recommendations to address them
- Focus solely on assessmentsno prescribing medications or creating treatment plans is required
Qualifications:
- Valid Nurse Practitioner license issued by the State of Michigan
- Active CPR certification (BLS)
- Board Certification through AANP or ANCC
- Minimum of 6 months recent Nurse Practitioner experience (preferred)
- Master of Science in Nursing (MSN) from an accredited institution
You'll be compensated at $80$90 per evaluation with a mileage stipend and compensation for administrative time, acknowledging both your expertise and time invested in patient care.
