Job Description
Take on a dynamic role as a Nurse Practitioner conducting home health risk assessments for Medicare Advantage members across Cumberland County. You will have the flexibility to build your own schedule, working per diem and visiting patients in their homes in areas such as Fayetteville, Hope Mills, Spring Lake, and Eastover. Each assessment typically takes about 45 minutes and may focus on annual health evaluations or post-discharge follow-ups.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduled shifts to fit your preferences
- Minimum weekly or bi-weekly hourly commitment tailored to your availability
Key Responsibilities:
- Perform comprehensive annual home health risk assessments for Medicare Advantage plan members
- Review and assess prior/current medical history, ongoing medical conditions, and medication usage
- Deliver clear education to patients about their health conditions and prescribed medications
- Identify care gaps, address health risks, and provide actionable recommendations to support improved outcomes
- No requirements to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Current Basic Life Support (BLS) CPR certification
- Board certified through AANP or ANCC
- At least 6 months of recent experience as a Nurse Practitioner (preferred)
- Master of Science in Nursing (MSN) from an accredited institution
