Step into a flexible per diem opportunity as a Nurse Practitioner specializing in home health risk assessments across Cumberland County, NC. This role focuses on delivering comprehensive annual and post-discharge health evaluations for Medicare Advantage plan members in diverse locations, including Fayetteville, Hope Mills, Spring Lake, and Eastover.
Schedule:
- Flexible shifts available Monday through Sunday
- Enjoy autonomy to set your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Primary Responsibilities:
- Conduct thorough home-based risk assessments, with each visit lasting approximately 45 minutes
- Review and evaluate past and current medical histories, ongoing health conditions, and detailed medication usage
- Educate patients on their specific medical conditions and prescribed medications to enhance understanding and compliance
- Identify gaps in care and make informed recommendations for addressing these concerns
- Focus solely on assessment with no prescribing or development of treatment plans
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board Certification as an NP (AANP or ANCC)
- Current CPR certification (BLS)
- MSN degree from an accredited institution
- Preferred: At least 6 months of recent NP experience, ideally in home health
This position offers compensation of $80-$90 per evaluation, complemented by a mileage stipend and reimbursement for administrative time.
