Experienced Nurse Practitioners with a passion for home health are invited to make a direct impact through comprehensive risk assessments for Medicare Advantage members in Washington County, AR. This per diem role offers the flexibility of creating your own schedule with shifts available Monday through Sunday. Enjoy autonomy while meeting the home health needs of individuals in Fayetteville, Springdale, Farmington, and nearby communities.

Schedule:

  • Flexible shifts: MondaySunday
  • Create your own schedule
  • Set your preferred weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct annual risk assessments in members homes, focusing on medical history, current conditions, and medication review
  • Educate patients about their conditions and medications
  • Identify care gaps and recommend actionable solutions
  • Each visit averages approximately 45 minutes; nature of visits may vary (annual assessments or post-discharge)
  • No requirement to create treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in Arkansas
  • Board Certification through AANP or ANCC is required
  • Current CPR (BLS) certification
  • Minimum of 6 months of recent nurse practitioner experience preferred
  • Graduate of an accredited MSN program
