Job Description

Make a positive impact by delivering personalized healthcare to individuals in their homes as a Nurse Practitioner specializing in Home Health Risk Assessments. This per diem opportunity allows for true flexibilitydesign your own schedule with shifts available any day of the week, Monday through Sunday. Minimum weekly or bi-weekly hourly commitments ensure you can tailor your workload to fit your lifestyle. Assignments will take you throughout Benton County, with home visits in Rogers, Bentonville, Bella Vista, and other nearby communities.

Key responsibilities include:

  • Conducting annual home health risk assessments for Medicare Advantage plan members
  • Reviewing and evaluating patient medical history, current conditions, and medications
  • Providing clear patient education on identified health issues and prescribed medications
  • Identifying care gaps and recommending resolutions
  • Focusing on evaluation and education, with no treatment planning or prescribing required

Schedule:

  • Flexible shifts available MondaySunday
  • Select and build your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Desired qualifications:

  • Nurse Practitioner license in Arkansas
  • Graduate of an accredited MSN program
  • Board Certification through AANP or ANCC
  • Current CPR certification (BLS)
  • At least 6 months of recent experience as an NP (preferred)
